Have you been taking your measurements? maybe the scale is not budging but you are losing inches?

I definitely think you should try upping your exercise program.

No to all those questions.

Keep doing what you are doing since you must have been doing it right since you have lost so much. Maybe your body is just "resting" or taking a break.

Up the exercise, track your measurements, keep drinking your water, eating right, staying away from known stallers and your whoosh will come.
